Admission Details Before Joining Four Seasons Care Home, Inc.

Whether it's a daily activity work or a greater degree of help, we're here to provide you with the assistance you need. Please go through our admission agreement carefully. Assistive Community Care Services (ACCS) program is an option for Vermont residents who are currently enrolled in the community Medicaid program and are residing in or are moving into Level III residential care home.

Please note that, to be considered for admission, you need to have been approved for VT Medicaid. Applications that are still pending won’t qualify.  If you do not qualify for Medicaid we also accept private pay.
